"Should I ignore the SIB to avoid reinforcing it?"
No — not for sensory-maintained SIB. Ignoring does not reduce sensory needs (which are not socially mediated). Ignoring allows injury to continue. The correct approach is to provide safer sensory alternatives. If SIB is attention-maintained (confirmed by FBA), planned ignoring may be part of a professional plan — but never without a replacement behavior trained simultaneously.
"Will the protective gear become permanent?"
No. Protective equipment is a safety bridge — used while therapeutic alternatives are being established. As replacement behaviors strengthen, protective gear needs typically decrease. Your BCBA/OT will create a fading plan as part of the intervention.
"Is it safe to let my child bite a chewy tool as hard as they want?"
Yes — when the chewy is rated for aggressive chewing and is not damaged. The intensity is often necessary to meet the oral proprioceptive need. Trying to limit bite force defeats the purpose. Inspect regularly for damage and replace when worn.
"My child only uses the alternative material when prompted. Is that progress?"
Yes — absolutely. Prompted use is the first phase of skill acquisition. The prompting hierarchy: physical → gesture → verbal → independent. Celebrate prompted use enthusiastically. Independence comes after prompted use is well-established.
"How do I know which function is driving the SIB?"
This requires a Functional Behavior Assessment by a BCBA. Clue: if SIB occurs alone with no social audience, it's likely sensory-maintained. If it increases during demands, escape may be a function. Most require professional FBA for certainty.
"At what point should medication be considered?"
Medication is sometimes considered for severe SIB when behavioral and sensory interventions have been implemented consistently and are insufficient. Always requires consultation with a developmental pediatrician specializing in autism. Medication is always adjunct to — never replacement for — behavioral and sensory intervention.
"My child has been doing this for years. Is it too late?"
It is never too late. SIB can be addressed at any age with appropriate intervention. Older children and adolescents may require different approaches, but the evidence base for behavioral and sensory intervention spans all ages through adulthood.
"Can diet changes help?"
