"Session abandonment is not failure — it is data. Every difficult session teaches you something." Here are the 7 problems Pinnacle OTs hear most frequently from families in the first four weeks — and the clinical fix for each.
Problem 1: Child refuses to touch the therapy putty
Why: Tactile sensitivity — the texture is aversive. Fix: Start with the most neutral material (building toys or pegboard). Use gloves for initial putty contact. Progress gradually — don't force tactile.
Problem 2: Child uses whole fist on tweezers instead of pincer
Why: Pincer grasp isn't yet habitual; reverts to easier pattern under demand. Fix: Go to larger tongs (kitchen salad tongs) where whole-hand IS the correct grip. Build confidence at that level first.
Problem 3: Child gets frustrated after 2 minutes and quits
Why: Materials are too difficult OR session starts without adequate pairing. Fix: Drop to the easiest version of the material. Extend the invitation phase before introducing demand.
Problem 4: Scissors crumple the paper instead of cutting
Why: Scissors too hard; paper too flexible; wrist position rotated inward. Fix: Switch to spring-loaded self-opening scissors + cardstock. Check "thumbs up" wrist position.
Problem 5: Child rushes through the pegboard without precision
Why: Speed preferred to accuracy; no quality incentive. Fix: Change the goal — "place 10 pegs without knocking any over." Precision incentive, not speed.
Problem 6: Child's hand fatigues in under 5 minutes of putty work
Why: Resistance is too high; intrinsic muscles are genuinely weak. Fix: Move to softer putty. 3 minutes of soft putty daily > 1 minute of firm putty.
Problem 7: Child refuses the dressing frame but not other materials
Why: Dressing is associated with stressful morning routines. Fix: Introduce the frame at afternoon play time. Call it "the button game" — never "practice." Use a doll's clothing first.
