Before Your Child Sits Down — the Environment Must Be Ready
A child in a fear state is hyper-attuned to their environment. The wrong lighting, a cold seat, or a rushed caregiver can activate the threat response before the session even begins. Set the stage once. Set it right. Spatial and sensory preparation prevents 80% of session failures; the environment must be set BEFORE the child enters — not adjusted mid-session.
1. The Footstool — Position
Height: child's feet must rest flat, knees at or above hip level (approx. 90–110° hip angle). Material: non-slip surface, stable under weight. Correct squatting angle relaxes the puborectalis muscle, reducing physical effort by up to 30%. DIY: a stack of firm books wrapped in a non-slip mat works identically.

What You're Doing

Therapeutic dosage matters as much as technique quality. Frequency, variation, and knowing the satiation point are the three levers of this step.
Target Schedule: 3× daily — after each meal (breakfast, lunch, dinner). Gastrocolic reflex is strongest 20–30 minutes after eating — this is your highest-probability window. Minimum: 1× daily (after the largest meal).

Every common obstacle has a specific solution. This is not a reason to abandon the protocol — it's an invitation to adapt it.

Return to the exposure hierarchy. Start at the step BELOW where resistance occurs. Sitting on closed lid. Standing in bathroom. Talking about the toilet. Wherever there's zero resistance — that's your current rung. Daily practice happens there.

🟡 "My child sits but is clearly holding the whole time"
This is normal for weeks 1–4. The sitting itself is therapeutic. Focus on breathing tools. Do NOT comment on whether they're holding. Add a particularly engaging item to the kit.

🟡 "We had a successful BM last week but nothing this week"
Regression is part of the pattern. The fear cycle has memory. Maintain the protocol exactly. Review fibre supplement compliance. Check if stool has hardened (track in chart).

Completely normal as a starting point. Fibre + toilet stool + engagement kit for sitting in nappy first. The nappy can be loosened over sessions, then opened, then removed. Graduated exposure works even here.

Q1: How long will this take?
There is no universal timeline. Most families see measurable improvement within 4–8 weeks of consistent combined protocol (soft stool + behavioural intervention). Children who have withheld for years may require 3–6 months. The cycle took time to form; it takes time to unform. Consistency, not speed, predicts success.
Q2: Do I need to use all 9 materials?
Not necessarily all simultaneously. The essential foundation is: (1) toilet stool for positioning, (2) stool softener/fibre per medical guidance, (3) scheduled sitting with engagement kit, (4) reward system. The other 5 materials support and accelerate but the foundation 4 are non-negotiable.
Q3: My child is 8. Is it too late?
No. Children of any age can break this cycle with appropriate intervention. Older children require privacy considerations, age-appropriate social stories, and more sophisticated reward systems — but the physiological principles are identical. The later you intervene, the more entrenched the cycle, but the more cognitive capacity the child brings to understanding.
Q4: Should I use laxatives or stool softeners?
This is a medical decision made by your paediatrician or paediatric GI specialist. Polyethylene glycol (PEG/Miralax) is the most commonly recommended first-line agent. The key principle: stool must be kept consistently soft enough that bowel movements are physically non-painful. This is the medical foundation on which behavioural intervention is built.
Q5: My child has autism. Is this approach appropriate?

A privilege? The reinforcement must be identified by the child, not selected by the parent based on what "should" work.
